How to create a quiz with Microsoft Forms

Microsoft Forms makes it easy for you to create questionnaire forms to collect customer feedback, and here’s how to use the service.

Depending on your business, sometimes you will need to conduct a survey to engage with your customers, promote something new, or measure your customer satisfaction in order to improve a specific product or service, and that’s when Microsoft Forms is useful.

Microsoft Forms is a freemium service that makes it easy to create a quiz with multiple choice, text, scoring, likert, and net promoter score questions that you can invite others to answer to collect feedback from anywhere using any web browser or mobile device. The service also offers real-time results, analytics to examine responses, and more.

You’ll need a Microsoft 365 or corporate subscription to access premium features, but the free version includes all of the basic tools to create a shape like this.

In this guide, you will learn the steps to create a questionnaire form to collect feedback from people using Microsoft forms. In addition, we look at the steps to take to review participant responses.

To create a custom poll to ask for feedback, use the following steps:

  1. Open Microsoft forms online.
  2. Sign in to your Microsoft account (if applicable).
  3. Click on the New button from.Create a new questionnaire form

  5. (Optional) Click the Image button to insert an image for the form using Bing, OneDrive or manually upload.
  6. Click on new Add button.
  7. Select the type response you want to get from users completing the survey. The options available include:
    • Choice – adds a multiple choice question.
    • Text – adds a question that requires a written answer.
    • Evaluation – adds a question asking for a rating, and you can use stars or numbers for the symbols.
    • Date – adds a question asking for a specific date.
    • Ranking – adds a question asking to sort the answers by ranking.
    • Likert – adds a question that has 5-7 options with choices ranging from strongly agree to strongly disagree.
    • Net Promoter Score – adds a question to measure customer experience by asking users to offer a scale from 0 to 10 on something (usually the recommendation).

  9. Repeat steps no 7 and 8 to add more questions to the form.
  10. (Optional) Click the down-arrow button, while in the “add new” question menu, and select Section option to separate the content.
  11. Click on the button Theme in the upper right corner to select a background image and color scheme for the form.

  12. Click on the Preview button in the upper right corner to consult the complete questionnaire before publishing it.
  13. Click the back button in the upper left corner.

  15. Select the options for the form:
    • Accept the responses.
    • Start date.
    • End date.
    • Email notification of each response.
    • Personalize the thank you message.
      Quick Note: The free version of the forms or the available with Microsoft 365 Personal or Family does not include the “Who can fill out this form” setting to record participant names. You will need a corporate subscription to access this option and others.Survey optionsQuestionnaire options

  17. Select the option link.
  18. Click on the button Copy.

Once you have completed the steps, you can paste and send the link to anyone to collect responses via email or social network. In addition to copying the shareable link, you can get a QR code that recipients scan with their phones to access the form. In addition, there is an embed option to copy a code to embed on a web page, or you can even select the email option, which just copy the link to the clipboard and opens the client e -mail by default to compose a message to send the poll.

Alternatively, Microsoft Forms also offers a choice in the Settings menu to print the questionnaire, instead of creating a link, if you prefer to get a hard copy of the survey responses.

If you want to delete a form while in Microsoft Forms, select the “All my forms” option from the lower right side, then click the ellipsis menu button in the form and select the option Remove.

Microsoft Forms also offers questionnaire templates and other types of forms that you can customize to quickly create and publish the form you need.

To create a questionnaire form with a template, use the following steps:

  1. Open Microsoft forms online.
  2. Sign in to your Microsoft account (if applicable).
  3. Click on the More button models.
  4. Select one of the available models. For example, poll on customer feedback.

  6. (Optional) Click the Image button to insert an image for the form using Bing, OneDrive or manually upload.
  7. Review and edit each question as needed.

  8. (Optional) Click the new Add button to create more questionnaire questions.

  10. Click on the Preview button in the upper right corner to consult the complete questionnaire before publishing it.
  11. Click the back button in the upper left corner.
  12. Click on the More button settings (ellipsis) in the upper right corner and select the option Settings.
  13. Select the options for the form:
    • Accept the responses.
    • Start date.
    • End date.
    • Email notification of each response.

  16. Click on the button Copy.

Once you’ve completed the steps, you can send the link to anyone you want to get their feedback.

Once you have published the survey, you can check the responses from the form.

To view survey responses on Microsoft forms, use the following steps:

  1. Open Microsoft forms online.
  2. Sign in to your Microsoft account (if applicable).

  5. Confirm the number of people who submitted their survey responses and the average time it took to complete the survey.
  6. Review the responses to the questionnaire.

  8. Review the responses for each submission.

Once you complete the steps, you will have a better understanding of the participants, and you can use that data to improve the products or services you provide.
